Surface Treatment and Osseointegration Profile

TiUnite is the Nobel Biocare proprietary surface, a moderately rough titanium oxide layer produced by anodisation in a phosphate-rich electrolyte. The result is a thicker oxide than machined titanium (0.6–10 µm vs <0.2 µm for machined), with a porous topography at the micro-scale that supports rapid bone-to-implant contact.

Clinical evidence is substantial across the Nobel TiUnite ranges. Glauser et al. (2007) reported 96.0% TiUnite implant survival at 5 years across 1,920 implants in 506 patients, including immediate-load and posterior cases. The 10-year Brånemark Novum and equivalent cohorts maintain the 95–97% survival range, with the Maló All-on-4 cohort at the highest end (94.8% mandibular at 20 years).

The TiUnite hydrophilicity profile is more conservative than Straumann's SLActive, Nobel Biocare positioned the surface as a stable, well-documented chemistry rather than chasing the high-surface-energy preserved-storage approach. For the clinical case, TiUnite has the longest single-surface long-term cohort outside of SLA and is the surface most commonly published in immediate-load full-arch literature.

Drill Sequence and Torque Profile

Nobel Biocare uses brand-specific drill kits, different for the parallel-walled Replace Conical Connection, the tapered NobelActive, and the BCC ranges. Standard saline irrigation rate of 50 ml/min. Target insertion torque is 35–45 Ncm for delayed loading; immediate loading requires 45–70 Ncm depending on the protocol (NobelActive achieves higher torque in tapered placement).

Recommended Primary Stability Threshold

ISQ 65+ for delayed loading; ISQ 70+ for immediate loading; the All-on-4 protocol requires the four-implant Insertion-Torque Composite to meet documented gates (Maló protocol).

Indications and Contraindications

Indications. Single-tooth, multi-unit, full-arch (All-on-4 / All-on-6), immediate placement, immediate load, zygomatic-anchor pterygoid combinations.

Relative contraindications. Same general patient profile as Straumann, heavy bruxism, irradiated bone, uncontrolled diabetes, active periodontitis, smoking above ten cigarettes per day.

Absolute contraindications. Active head-and-neck cancer with current radiation; bisphosphonate therapy without coordinated cessation; severe untreated systemic disease.
